Since the passage of the national Clean Air Act in 1970, and promulgation of standards under that Act, ambient air quality has measurably improved in many regions of the United States. Comparing the major sources of air pollution as measured in 1970 to those in 1992 gives some clues as to changing patterns of air pollution. Table 5-1 shows composite estimates for the United States, obviously subject to wide local variation. The variation is greater worldwide. Volcanic eruptions, for example, have substantially increased atmospheric particulates, sometimes for many years.

Untreated medical wastes have been specifically excluded from most landfill, mass burn, and refuse derived fuel (RDF) facilities. The irony is that there has been no evidence of infection from medical waste spread to the community. However, in 1997 there was a report from the state of Washington of the possible occupational transmission of tuberculosis (3 cases and 13 conversions) to employees of a commercial medical waste facility. Apparently, filters on the shredding device became clogged, resulting in the aerosol bypassing the filter and infecting the workers.

This approach has worked quite well for more than twenty years. One drawback is the production of nitrous oxide (laughing gas), which is itself a greenhouse gas.
